Focused on a wellness-based model, the health center provides services to support the bodies, minds and emotional well-being of students.

Monday-Friday: 8:30 a.m. – 4 p.m.

The health center is closed on weekends, school breaks, holidays and summer break.

Services through the Student Health Center are available to all enrolled students free of charge. The Student Health Center nurse practitioner will make referrals to off-campus healthcare service providers as needed. Students are financially responsible for medical treatments that are received off-campus.

Students are seen on a walk-in basis during clinic hours.

Medical Instructions for New Students 

New students entering Montreat College are required to complete a medical self-history form and submit childhood vaccine records. 

These records should be submitted once an incoming student has committed to attend the university, but no later than July 1st for Fall enrollment, and by January 1st for Spring enrollment. The records can be submitted online via the Map portal. 

**Please note that students cannot move into the residence halls until all forms are submitted and cleared by the Director of Health Services.

Please be aware that it is the student’s responsibility to advise any faculty, instructors, or program providers of any pertinent medical conditions and/or any limitations on your activities by your physician.

**All sports physical forms and athletic related forms are turned into the coach or athletic trainer**

The Student Health Center is here to support your health needs. To ensure a smooth start, please submit your immunization records on the portal before arriving on campus. Our goal is to have this information prior to your arrival. The records can be submitted online via the Map portal.

  2. Incomplete Requirements: If you can’t meet all requirements before arrival, we can assist you during your first weeks here, though you may be required to travel to the county health department on your own time and expense to receive the required vaccines.

Immunization Records

Translated to English

  • Include your name, student ID number, date of birth (MM/DD/YYYY).
  • Provide the doctor’s office name, address, phone number.
  • List the date of each immunization received.
  • Lab and Radiology Reports:
    • Must be official and complete from the lab or radiologist.
    • Include your name, birth date, and the lab/radiologist’s contact information.
    • Lab reports should have numeric values for normal ranges and your results.

Required Immunizations

  • Tetanus: 3 doses at any time; Tdap (Tetanus, diptheria, pertussis) booster needed if entering 7th grade or by age 12. US brands: Boostrix, Adacel.
  • Measles: 2 doses after first birthday, at least 1 month apart.
  • Mumps: 2 doses after first birthday, at least 1 month apart.
  • Rubella: 1 dose after first birthday.
  • Hepatitis B: 3 doses required for students born after June 30, 1994; administered at 0, 1 month, 6 months. Lab reports cannot substitute for this requirement.
  • Polio: 3 doses required if under 18.
  • Varicella: 1 dose after first birthday for those born after April 1, 2001. Lab reports can substitute for this requirement.
  • Meningitis ACWY: 2 doses. If you received the first dose on or after your 16th birthday, you are not required to receive a 2nd dose.
  • Tuberculosis Testing: Within 12 months prior to the first day of class:
    • Negative TB skin test from your doctor translated to English
    • Negative IGRA (Interferon-Gamma Release Assay) lab report.

Ensure all reports meet the above requirements for completeness and accuracy.

Immunizations

All undergraduate students, full-time commuter day students, and part-time commuter days students taking more than four credit hours must submit proof of receipt of all required immunizations via the online MAP portal. According to North Carolina Law (G.S> 130A-155.1), students who do not comply will be withdrawn from classes without credit and removed from housing.

Acceptable immunization records may be requested and obtained from various sources, including the State Health Department, family physicians, military records, or the World Health Organization. Proof of immunization records must include dates of vaccine administration and the student’s name.

Insurance

All students are required to have their own health insurance while enrolled at Montreat College. All domestic students will be enrolled in a supplemental insurance policy but it is not meant to replace primary health insurance. International students will be automatically enrolled in an annual health insurance plan that is covered by their international student fee.

**Please note that Medicaid can only be used as primary health insurance in the state it is issued in. **

Confidentiality

The Student Health Center complies with federal and state laws regarding the confidentiality and security of medical records, including HIPAA. Medical records are confidential and can only be released with legal permission or patient authorization. They are provided to the patient only, unless it risks the patient’s safety or others. This applies to records from other health care providers as well.

  • Authorization: A written request is required to release medical information, except for:
    • Reporting certain communicable diseases to the NC Department of Health and Human Services.
    • Complying with a court order.
    • Providing immunization records to health departments, other colleges, or the patient’s physician
