Student Health Services Privacy Policy

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Any changes in practices will be posted outside Student Health Services.
1. Student Health Services (SHS) will maintain the confidentiality of students’ Personal Health Information (PHI) as outlined below.
2. SHS will obtain a student’s written consent before releasing any Personal Health Information (PHI). Students will be able to specify to whom their PHI is shared. This protection extends to any PHI, which is oral, written, or electronic. Students have the right to request a restriction of disclosures of PHI by submitting a request in writing. Exceptions allowed by law include:
a. Disclosure to parents or legal guardians if student is a minor.
b. Individuals involved directly in student’s medical care.
c. Law enforcement—in response to a court order, subpoena, warrant or similar process.
d. Public Health—disclosure of student’s PHI to federal, state or local authorities or entities charged with preventing or controlling disease, injury, or health activities.
e. Serious Threat—SHS may disclose PHI to appropriate when necessary to prevent serious threat to a student’s health or safety. In the academic setting, this may include Dean of Student Services, Dean of Academics, or Campus Police.
3. As mandated by law, students may gain access to PHI upon written request. SHS will provide PHI within one week of written request.
4. As provided by law, the student may register a complaint (in writing) to the Department of Health and Human Services (HHS) at http://www.hhs.gov/ocr/privacyhowtofile.htm and to SHS if he/she believes their rights have been violated.
