Fill Out The Verification Worksheet
Only If You Have Been Requested To Do So


The DOE selects about 30% of SARs for verification.  The Financial Aid Office may also choose a FAFSA for verification in order to resolve any questionable or conflicting information.

Please complete and submit the following by mail or fax so we may verify your FAFSA information:

  • A signed copy of your and your spouse's 1040 federal tax form(s)
    • Both must sign
    • only need 1040 or 1040EZ; (do not need attachments, schedules, or addendums) - see 1040 blank example below
  • A signed copy of your parents' 1040 federal tax form(s) if you are a dependent student
    • Dependency status is determined by your FAFSA results
    • only need 1040 or 1040EZ; (do not need attachments, schedules, or addendums) - see 1040 blank example below
  • A copy of your and your spouse's/parent's W2 form(s) if any of your FAFSA answers came off of your W2 information
  • The Verification Worksheet (see chart below) must be signed.
  • Copies of any other documentation that was used when filling out your FAFSA
    • Copies of social security statement, etc.
